By clicking the box below, you acknowledge the submission of this form as a contractual agreement between you (the guardian) and MDAH (the academy). For this Application for Admissions to be considered complete, all applicable fees must be received by MDAH, consisting of the Application Fee and Materials Fee. Once all paperwork and fees are received, MDAH will notify you of our admissions decision via your preferred method of paperwork delivery noted in this application within two (2) weeks. In this correspondence, you will find requests for documentation required by MDAH, The State of South Carolina, and the Department of Social Services. You will have two (2) weeks to provide this documentation to MDAH. NO STUDENT IS CONSIDERED ENROLLED IN THE SCHOOL UNTIL THE FEES, APPLICATION FOR ADMISSION, AND OTHER REQUIRED DOCUMENTATION ARE RECEIVED. Should any part of this process be omitted or timelines not adhered to, MDAH reserves the right to reverse the admission decision and open the space for the next student on the waiting list. If the school has full enrollment, a waiting list will be established and applications on file will be reviewed in the order they appear on the list. As space becomes available, the Administrator will notify parents and the above process will be followed to procure the child a space in enrollment. Tuition is to be paid in the method selected on this application. Payment of tuition should follow the guidelines set forth in the Parent Handbook; failure to pay tuition may result in suspension or termination per the handbook guidelines.
